How to call Thuraya satellite phone from the USA and Canada?

Calling Thuraya satellite phones from the United States and/or Canada explained:

A) 011 - US exit code; must be dialed first for all international calls made from the USA or Canada
B) 882 16 - country code for the Thuraya network
C) Thuraya phone number - 8 digits
D) A+B+C: CANADA or US to Thuraya international dialing format: 011 + 882 16 + ???? ????
